Report Residential Vacate Order

If your home has been issued a Vacate Order, the Back Home Unit (BHU) of the Mayor’s Office of Housing Recovery Operations (HRO) can help. We can provide you with information, next steps, and resource coordination.

If you were affected, please fill out this Intake Form. You may also fill it out for someone else with their consent. If you do, please answer the questions as if you were that person. After you submit the form, a BHU staff member will contact you within one business day to help you.

How the Back Home Unit can assist

  • Provide information and next steps if your home is not safe to live in.
  • Help you understand impacts of Vacate Orders issued by city agencies.
  • Coordinate resources and follow-up through the Back Home Unit (BHU).

You may complete the Intake Form for yourself or on behalf of someone else. If you do, please answer the questions as if you were that person.

Before You Begin

Sign in with NYC.ID

To submit your information, you must sign in through NYC.ID, NYC’s secure authentication application. NYC.ID allows you to access NYC services, including the Back Home Portal, using your existing Google, Yahoo, Microsoft, or LinkedIn account. Get Help by Phone or Email

If you cannot or do not wish to sign in online or are using assistive technology currently incompatible with this form, you can reach a BHU staff member directly by phone 212-389-6670 or email backhome@recovery.nyc.gov.

Information that may be helpful to have

  • Address of the home that is not safe to live in or is subject to a Vacate Order.
  • Contact information for you, or for the person you are completing the form for.
  • Date the fire, disaster event, or other emergency occurred, if known.
  • Any notices or communications you have received about a Vacate Order.

You can still complete the Intake Form even if you do not have all of this information available. A BHU representative can follow up to clarify details.

What happens after you submit the form?

Submission received

Your Intake Form is received by the Back Home Unit (BHU) at NYC Housing Recovery Operations.

Follow-up

After you submit the form, a BHU staff member will contact you within one business day to help you.

Information and coordination

BHU can assist you with information, next steps, and resource coordination related to your situation.
